Health Forms & Orthopedics


All Ursuline Student Athletes must have a valid physical form on file in order to be eligible to tryout for a team. Physicals are good for one year. Click here to obtain the form to be filled out by your physician.

Athletes must also complete an interval health history form at the start of each season. The form should be brought to the school nurse. Click here to obtain the form.

***Due to the COVID-19 pandemic and the impact that it is having on our health care providers, The Department has released a memo to provide direction to schools when students are delayed in obtaining required health exams, along with information on the required health exam form. (attachments)  

  • Continue to accept any physical examination form conducted on or before January 30, 2021 and allow parents/guardians additional time to provide the completed exam form to the school;
  • Student athletes can participate in the fall 2020 sports season without a current health exam if they meet certain criteria; and
  • For the past two school years, the Department has encouraged schools to accept a health exam form regardless of the form it was completed on.  Beginning January 31, 2021- physical examinations for school are to be documented on the NYS Required Health Examination Form (attached here) 




Northwell Health’s Orthopedic service line is now available for Ursuline athletes. Dr. Louis McIntyre is an experienced orthopedic surgeon with offices in Sleepy Hollow, Valhalla and Manhattan, and has been serving the Westchester community for over 20 years and is the overseeing physician for the Ursuline school's certified athletic trainer. Dr. McIntyre is also a personal member of the Ursuline community with his daughters being students of Ursuline. 

The Orthopedic Fast Track Program (844-966-7846) allows parents to speak with a live person and offers priority scheduling for athletes.  The Fast Track team is available to schedule appointments with Dr. McIntyre Monday - Thursday (7am-8:30pm), Friday (7am-7pm), and Saturday (8am-4pm).
