PRIOR TO TRYING OUT FOR A TEAM
All Ursuline Student Athletes must have a valid physical form on file in order to be eligible to tryout for a team. Physicals are good for one year. Click here to obtain the form to be filled out by your physician.
Athletes must also complete an interval health history form at the start of each season. The form should be brought to the school nurse. Click here to download the Interval Health Form. Please remember that an interval health history form is not the same as an annual physical.
Individuals who test positive for COVID-19 should not participate in interscholastic athletics until they are cleared by a Health Care Provider. Therefore, any athlete who tests positive for COVID-19 or/and tested positive for COVID-19 antibodies is required to complete the “TUS COVID-19 Return to Athletics - Health Care Provider Clearance Form.”
Athletes who participate in athletics and have had a positive COVID-19 test 14-days prior to the start date of the season, or an athlete who is in-season and tests positive for COVID-19, will be required to complete this Return to Athletics Protocol under the supervision of the Certified Athletic Trainer.
Please complete the consent form so that information can be shared with the appropriate individuals to ensure your child's safety. Any information provided will remain confidential.
Important Note: The Ursuline Athletic Director, Mrs. Saxton, at firstname.lastname@example.org and Nurse, Mrs. Bruno at email@example.com must be notified of each case and are required to clear each student. This review to clear may take time so it is imperative to get the necessary documentation, as soon as possible so that this process can be completed for clearance. TRAVEL & COVID TESTING. Any student-athletes who travel during the Easter Break are to provide a negative PCR test to return to sports during the break. By presenting this negative test you will not have to re-test on April 12 to return to classes. Please send test results to firstname.lastname@example.org
NORTHWELL HEALTH ORTHOPEDIC LINE
Northwell Health’s Orthopedic service line is now available for Ursuline athletes. The Orthopedic Fast Track Program (844-966-7846) allows parents to speak with a live person and offers priority scheduling for athletes. The Fast Track team is available to schedule appointments Monday - Thursday (7am-8:30pm), Friday (7am-7pm), and Saturday (8am-4pm).
