HEALTH FORMS - PRIOR TO TRYING OUT FOR A TEAM

All Ursuline Student Athletes must have a valid physical form on file in order to be eligible to tryout for a team. Physicals are good for one year. Click here to obtain the form to be filled out by your physician. Please scan all updated physicals to health@ursulinenewrochelle.org

Athletes must also complete an interval health history form at the start of each season. The form should be brought to the school nurse. Click here to download the Interval Health Form. Please remember that an interval health history form is not the same as an annual physical.

 

TUS COVID-19 Return to Athletics - Health Care Provider Clearance Form.

Individuals who test positive for COVID-19 are prohibited from participating in interscholastic athletics until they are cleared by a Health Care Provider. Therefore, any athlete who tests positive for COVID-19 or/and tested positive for COVID-19 antibodies is required to complete the “TUS COVID-19 Return to Athletics - Health Care Provider Clearance Form” updated January 13, 2022.

Please schedule an appointment and have your primary care doctor fill out the form when the student-athlete has completed isolation and is feeling better. (They cannot be cleared prior to the 10-day isolation period). For example, if a student-athlete began isolating on the 1st of the month, she can be cleared on the 11th of the month. 

 

NORTHWELL HEALTH ORTHOPEDIC LINE

Northwell Health’s Orthopedic service line is now available for Ursuline athletes. The Orthopedic Fast Track Program (844-966-7846) allows parents to speak with a live person and offers priority scheduling for athletes.  The Fast Track team is available to schedule appointments Monday - Thursday (7am-8:30pm), Friday (7am-7pm), and Saturday (8am-4pm).
